Our Complex Care Folders are designed to bring everything together in one organised, easy-to-access place. They provide structured clinical documentation to support children with significant health or medical needs. Developed by a Registered Nurse, these folders consolidate key information including medical history, care plans, medication management, risk management strategies, emergency protocols, key contacts and delegated care provider tasks. They ensure support workers and care providers have clear guidance to deliver safe and consistent care. They are especially valuable for families managing multiple providers, as they streamline communication and ensure everyone involved in care has access to accurate, up-to-date information. Complex Care Folders are practical, user-friendly, and ideal for use at home, during appointments, or in emergency situations, helping reduce stress and improve overall care provided. Complex Care Folders also provide clinical evidence to support safe delegation, care planning and appropriate NDIS funding decisions. Consultations will take place in the comfort of your home or via Telehealth and usually take a few hours to complete, after which all the information gathered will be collated a complex care folder will be completed. Prices are aligned with NDIS pricing agreement for a clinical nurse consultants and will only be charged for hours required to complete the final document.
