Service Description

We provide Individualised Care Plans for children with complex health needs. Developed by an experienced Registered Nurse, each Care Plan clearly outlines how and when care should be delivered, ensuring all supports are safe, consistent, and aligned with evidence based best practice. Care Plans include detailed instructions on daily care needs, clinical considerations, risk management, and specific interventions tailored to the individual. They are designed to support families, support workers and healthcare providers in delivering confident, high quality care. Our care plans also align with NDIS goals and funding, helping ensure the right level of support is clearly documented and understood. Consultations will take place in the comfort of your home or via Telehealth and usually take a few hours to complete, after which a specific care plan will be developed Prices are aligned with NDIS pricing agreement for a clinical nurse consultants and will only be charged for hours required to complete the final document.
